group personal accident Insurance is extended to provide for payment of certain defined benefits in the event of the Insured’s death or disablement by accidental Injury .

please ensure to make the following available with your claim:

Documents Required: NON FATAL CLAIMS ( Permanent Partial Disablement and Permanent Total Disablement – PPD & PTD)

  • Duly filed and signed Personal Accident claim form  
  • All IPD/OPD records & report of attending doctor as regard to cause of accident, nature, extent of injuries & Disablement period
  • Investigating report like Laboratory test, X-rays and other reports essential for confirmation of injuries.
  • Police reports, if any
  • MLC in case of Road Traffic accident
  • Disability certificate from Board of Doctors of Civil Hospital authorized to issue
  • Discharge summary from the hospital
  • Copy of Driving license of injured person in case of Road Traffic Accident
  • Incase medical reimbursement coverage taken in the policy, Original Medical Bills corresponding to doctor’s prescription should be produced  only if insured wish to avail the medical reimbursement
  • Confirmation from bank that injured person was member or bank & account holder along with confirmation supporting the deduction of insurance premium from account
  • Copy of Driving license of injured person in case of Road Traffic Accident
  • In case of Unnamed policy, undertaking from insured confirming the total number of members/employees as on the date of accident

 

Document required: FATAL CLAIMS (Death)

  • Duly filed and signed Personal Accident claim form  
  • All IPD/OPD records & report of attending doctor as regard to cause of accident, nature & extent of injuries, if hospitalized
  • MLC in case of Road Traffic accident
  • Certified copies of FIR lodged with Police authorities
  • Post Mortem report
  • Death certificate from municipal authorities
  • Copy of driving license of deceased, in case of Road Traffic Accident
  • Confirmation from insured that deceased person was member or bank & account holder along with confirmation supporting the deduction of insurance premium from account
  • Photo ID card of deceased i.e. either PAN card or Voter ID card or Driving License
  • Photo ID card of nominee either PAN card or Voter ID card or Driving License
  • Proof of insured/deceased – nominee relationship
  • In case of Unnamed policy, undertaking from insured confirming the total number of members/employees as on the date of accident
